A $25 non-refundable processing fee is required for each application to be paid at the time of intake interview.

Criteria For Admission

• Able to feed self.
• Not be a wanderer (individual who does not try to leave premises on regular basis).
• Be able to ambulate and/ or transfer with 1 person assist.
• Must have a current TB and Physical taken within 3 months prior to the start date.
• Clients must be non-combative.

**All clients will be assessed for appropriateness prior to admission.

Fee Schedule
(Effective October 1, 2007 - September 30, 2008)

Level I Independent	$750	Independent with Ambulation, toileting (continent) and self feeding
Level II Supervision	$800	Supervision with Ambulation, toileting schedule, needs cuing for re-direction (Alzheimer's/dementia) and/or uses wheelchair for mobility
Level III Assistance	$1,000	Requires assistance with transfers/walking, toileting assistance/changes and/or wandering/behavior that requires special attention

Daily/Saturday Rate:	$55.00 per day
Shower Fee:	$20.00 per shower (emergency only)
Incontinent care supplies (if client does not bring own supplies)	$1.00 each incident

 

Medicaid waiver services are not billable during periods of CLIENT hospitalization, long-term institutionalization or periods of suspension.

Rates are inclusive of Hawaii General Excise Tax.
